While states such as New Jersey, Arizona and New York are moving to cut their Medicaid enrollment, California’s program is experiencing its most significant expansion since the 1960s. A special waiver granted by the federal government called a “Bridge to Reform” will change eligibility rules and allow hundreds of thousands of Californians to enroll in Medi-Cal over the next two years, including previously excluded single adults. That does not include the estimated 2 million more Californians expected to enroll in Medi-Cal after the Patient Protection and Affordable Care Act allows Medicaid program expansion in 2014. The effects on California’s healthcare delivery will be profound. Although Medi-Cal rates are arguably better than providing charity care, hospitals, physician groups and county health plans will be under enormous pressure to provide and coordinate care for relatively sparse reimbursement.
