For many health
plan populations, a significant patient engagement gap exists between
base level outcomes involving unengaged patients, average adherence and
subjective feedback; and potential outcomes involving fully engaged
patients with optimal adherence and objective feedback. With Medicare
plans, this gap can further widen due to the greater level of resources
