A recent study published in
JAMA found that only 15.6% of physicians reported screening for all five
social determinant of health factors: food insecurity, housing
instability, utility needs, transportation needs, and interpersonal
violence. The infrastructure isn’t in place in most communities to address SDoH issues, and neither is the technology for health care providers to quickly, easily and effectively make referrals to community-based organizations. Physicians aren’t screening because they would be flagging problems without a way to address them. However, some social care and health care organizations are working together and because of these community networks, patients are being screened and SDoH issues are being addressed. During this webinar, Alliance for Better Health will talk about how it is working with health systems and payers in alternative payment arrangements, and providing their providers with SDoH data, allowing them to connect patients with community-based organizations to improve care and reduce costs. |
