Value-Based Care - in its spectrum of variations across the healthcare landscape - has emerged as the predominant paradigm for this new decade in collaborations and contracting between payers and providers. At the same time, Social Determinants of Health has emerged as the critical approach during this new decade in achieving improved outcomes and cost-effective allocation of resources for more vulnerable populations, particularly with Medicaid and Medicare Advantage programs.

Community-Based Organizations have become the key focal point in partnering with payers and providers to deliver SDoH components to member populations. As these partnerships widen and evolve, the demand is increasing for application of Value-Based care to these arrangements.
