| The Transforming Complex Care Project: SDOH Implications for ACOs | ||
|
There is growing
recognition that improving care for medically and socially complex
patients is essential for moderating healthcare spending growth and
improving health outcomes. A major challenge is that each complex
patient faces a combination of medical, behavioral health, and social
needs such as food, housing, and transportation that affect their
ability to initiate and complete their treatment plans. As a result,
health systems seeking to improve complex care must develop the
capabilities to individualize care plans for each patient in a way that
addresses these needs simultaneously. Transforming Complex Care (TCC), supported by the Robert Wood Johnson Foundation, is a six-site pilot demonstration aimed at refining and spreading effective complex care models. The TCC sites are assessing and addressing SDOH for populations with complex needs. TCC is beginning to offer lessons on how a diverse range of healthcare delivery systems can organize, develop, and scale care for complex populations. This webinar will offer lessons learned from the project with emphasis on implications for accountable care organizations. Key considerations to be addressed for organizations seeking to use SDOH data to improve patient care will include: (1) selecting and implementing SDOH assessment tools; (2) collecting patient-level information related to SDOH; (3) creating workflows to track and address patient needs; and (4) identifying community resources and tracking referrals.
