3 Keys to Integrated, Holistic Population Health Management
Population health management has transformed in just a few years
from a concept to a buzzword to, simply, what plans across the country
aim for as they seek information about their members and providers and
look for ways to maximize quality care and minimize costs. Just
because everybody's doing it, though, doesn't mean they're doing it
properly. Too often, "population health" is more about a specific
component - identifying pools of members with manageable health risks,
say, or building provider networks more on cost of services delivery
than on quality - and isn't focused on the value found in the sum of
its many parts. That approach has resulted in plans operating off what
is still very much a siloed population health strategy.
How do plans manage member risk and provider performance for the best
financial and health outcomes? How do they make the myriad components
of population health management work together for best effect, in
other words? This eBrief explores these questions and identifies three
ingredients for next-generation population health management.
