Life as we knew it took an abrupt shift in March. Thanks to COVID-19, we've seen schools move online, small businesses close, more than 5 million people lose health insurance due to furloughs and permanent job losses, and almost 1 in 5 American have moved to new locations.

At the same time, people with chronic conditions like COPD, Diabetes and Congestive Heart Failure, have been avoiding medical settings for routine tests and screenings for fear of exposing themselves to the virus. These are the 5% of the population that accounts for 50% of annual medical expenses in a normal year. By avoiding routine care, they may be getting sicker. Once life returns to normal, these members will also have a surge in demand for care.

Now more than ever, health plans need to focus on their population health management initiatives. Combining social determinants of health (SDOH) data with existing care management programs can bolster the effectiveness of those programs and help improve health outcomes.

Join us for an informative webinar on using SDOH data to turbo-charge population health management initiatives in a post-pandemic world.

Registration is complimentary for qualified applicants. Please join us on Thursday, October 1st at 2 PM Eastern. Click here for detailed information or to register.
