Showcasing Patient-centered Medical Home Models with Results

Virtually Integrated Models for Enhancing Primary Care
CareFirst has launched the CareFirst Primary Care Medical Home Program built upon a new set of incentives to primary care physicians that reinforce the central role of the primary care physician in helping members to understand and manage their health risks as well as guide their care when they experience major illness, especially involving chronic disease. Major elements of the program include how primary care physicians who volunteer to carry out the Program’s objectives are provided additional fee-based compensation and support to achieve results.

Multi-Payer Approach to a Patient Centered Medical Home Collaborative
WellPoint’s affiliated health plans participate in several multi-payer Patient Centered Medical Home (PCMH) pilot programs nationwide to test this emerging model for care delivery. The PCMH model emphasizes the core attributes of primary care, including access and availability, longitudinal relationships, comprehensiveness and coordination.
