Enabling Population Health within a PCMH through Advanced
Patient Focused Analytics
Chronic disease affects one in ten Americans and accounts
for 75% of the dollars spent on healthcare each
year. Compounding the problem is the fact that nearly half
the people with chronic disease are not receiving the care
required to keep them in optimal health. Targeting and
managing the chronically ill is the key to improving care
and reducing cost. As health plans look deeper at population
health management strategies, collaborating with network
providers with a PCMH model for ways to manage chronic
conditions will be a key to success.

Presenter Profile: Avery Ashby, Product Manager, MEDai
Avery Ashby, MS is the Product Manager responsible for the
Risk Navigator product suite for Elsevier/MEDai. Joining the
company recently, he brings more than eight years of payer
experience in Medical Informatics with a focus on Care
Management Analytics. Passionate about how informatics and
predictive modeling may be leveraged to bend the trend of
health care costs, improve quality of care, and positively
affect population health through effective implementation,
Avery is focused on providing tools to enable
clinicians/organizations to better manage the P4P/at-risk
environment.
Avery earned a Master of Science degree in Statistics at the
University of Tennessee - Knoxville. His work has been
recognized in several peer reviewed journal articles and
oral presentations at national conferences. He is also an
adjunct professor of Statistics at the University of
Tennessee - Chattanooga and currently is a co-chair for the
Population Health Management Evaluation workgroup with the
Care Continuum Alliance.
