Best Practices in Managing Advanced Illness though Population Health Management
 
Population Health has been an important component of stakeholder initiatives advancing innovation and transformation of healthcare to deliver improved outcomes, quality and value. Population Health News included this definition of population health in a recent issue quoting advisory board members:  “Population health is the ability to define and understand the health status of every individual patient and proactively deploy medical resources to support those patients, whether it is to push resource to them where they are, or effectively connect them to the optimal resource in a patient specific manner, accelerated by technology.”

Aetna's Compassionate Care Program has been addressing the advanced illness population for ten years, and involves trained, experienced case managers that provide case management education, support and resources to members, family and caregivers; pain and symptom management; facilitation of informed care decision making; and communication about understanding of prognosis. The program gives access to hospice care earlier and allows people to continue receiving curative care while in hospice. Aetna's nurse case managers receive special training in advanced illness and palliative care issues and frequently help address concerns that may be difficult to share in a face-to-face setting. Program results can also help reduce unnecessary costly hospitalizations and emergency room (ER) visits. Aetna typically experiences a drop of about $12,000 in health care costs for each person in their compassionate care program.
