North Country ACO & Mid-State Health Center's Story
 
Overview
As newly launched Medicare Shared Savings Program, Pioneer ACO and Commercial Accountable Care Collaborations move forward, current and future stakeholders can learn much by examining the experiences of those currently in the trenches, now launching and operating accountable care initiatives throughout the country.

Mid State Health Center, an independent, non-profit, federally qualified health center look-alike, with locations in Plymouth and Bristol, New Hampshire, is involved with North Country ACO, which now serves attributed Medicare Shared Savings Program participants, as well as the Central New Hampshire Health Partnership and New Hampshire Citizens Health Initiative ACO development that will bring together five delivery systems, multiple commercial health plans and state organizations.

Please join Sharon Beaty, Chief Executive Officer of Mid State Health Center on Thursday June 28th, 2012 at 1PM Eastern, as she provides an inside look at the environment, developmental challenges, operational issues, relevant dynamics, lessons learned and current considerations for Accountable Care with her organization.
