Atrius Health
has leveraged their Pioneer ACO as a platform for their Medicare
population health strategy. Atrius Health, the largest
physician-practice healthcare delivery system in New England, launched
their Pioneer ACO agreement in January 2012, ultimately creating one
model of care with full triple-aim accountability that provides
data-driven, stratified, best practice programs across their system.
This unified model addresses the medical and care managements needs of
their Medicare population, including fee-for-service Medicare patients
aligned through the Pioneer ACO model, members of their Medicare
Advantage product, or enrolled in the new financially integrated
programs for dual-eligibles. Providing a single model of care for the Medicare population, that is population-based, as opposed to siloing patients into different service models has allowed Atrius Health to think about their entire Medicare population in the same way clinically—with an approach that serves that entire population. Their approach also involves aligning all clinicians to work together, system-wide, on care delivery improvement.
