As stakeholders
grapple throughout the nation with managing readmissions for Medicare
populations, Aetna continues to make significant strides in coordinating
care for their Medicare Advantage members to improve quality and reduce
costs, through development of initiatives to address readmissions and
innovative collaborative relationships. Aetna's Medicare Provider Collaboration program includes an Aetna nurse case manager, "embedded" at the medical group, who helps coordinate care for Aetna Medicare Advantage members, and works face-to-face with the medical group clinical staff to assist in the coordination of the care that Aetna Medicare Advantage members receive from the medical group physicians. Aetna's comprehensive program is in place in a number of states and settings. Results from Aetna's provider collaboration with NovaHealth, a Maine IPA, were released in Health Affairs last fall, which yielded 56 percent fewer readmissions than unmanaged Medicare populations statewide. Aetna recently launched a new transitional care program to Aetna Medicare Advantage members in Texas, through a collaboration with Univita to help Aetna Medicare Advantage members transition smoothly from a hospital or skilled nursing facility to their home and prevent avoidable hospital readmissions. Similar programs administered by Aetna and Univita use evidence-based approaches and have been shown to reduce avoidable hospital readmissions significantly. Under this new program, upon admission to a participating hospital, specially trained nurses reach out to the member and their caregiver; engaging the member about their condition and discharge plan in order to determine the type of support the individual will need when they go home. If the member is admitted to a skilled nursing facility prior to returning home, the nurse will also engage the member while they are in the skilled nursing facility. After discharge to home, the nurse continues to offer and provide resource support to the member.
