Care
Oregon, a nonprofit Medicaid health plan, is transforming the delivery
of care through its work with the Institute for Healthcare Improvement
(IHI) as an Triple
Aim initiative
prototype site. By implementing two innovative programs designed to
optimize care for its most high-risk members, Care Oregon is
simultaneously improving enrollees' health, enhancing their health care
experience, and reducing the per capita cost of care. In their pursuit
of these three concurrent goals of the Triple Aim, Care Oregon and
other participating sites serve as models and inspiration for health
care systems across the globe.
