Changing Care Delivery
Join us
for a unique one-day opportunity March 5 at AHIP's second Summit
on Shared Accountability in Washington, D.C., as we showcase innovative
models that regional and national health plans have implemented to achieve
the three-part aim outlined in the National Quality Strategy – better care,
healthy people/healthy communities, and affordable care.
Numerous
health plans, in partnership with physicians, hospitals, and employers,
have implemented alternative care delivery and payment models that promote
accountability and help improve not only the quality and efficacy of care,
but also patient experiences. Early success in these models are helping to
change physician and hospital practices to become more patient-centered and
focus on quality across the care continuum rather than single encounters of
care.
It will
be a full day, packed with thoughtful discussion offering national and
local strategies that are changing care delivery models. You will hear
directly from practicing providers on how these new contracts are making a
difference in their practices.
